Community Transformation Pilot Program

We’re excited to introduce a new initiative led by the North East London Haemoglobinopathy Co-ordination Centre (HCC), recently funded to transform community care for Sickle Cell  (SCD) across the region. This pilot program aims to reshape how SCD care is provided by enhancing access to coordinated, holistic, and patient-centred services. 

Through this transformation, we’re creating a network of support that connects patients with leading practices close to home and broadens access to holistic and community care.

With this new model, we’re working towards a future where Sickle Cell patients receive seamless, comprehensive care that meets their unique needs and improves their quality of life. Join us as we embark on this journey to strengthen community care for Sickle Cell in North East London.
